ডেঙ্গু, টাইফয়েড ও দীর্ঘমেয়াদী জ্বর
জ্বর নিজে কোনো নির্দিষ্ট রোগ নয়; এটি শরীরে কোনো সংক্রমণ, প্রদাহ বা অন্য সমস্যার একটি উপসর্গ। ডেঙ্গু, টাইফয়েড এবং দীর্ঘদিন ধরে চলা বা বারবার ফিরে আসা জ্বরের কারণ আলাদা হতে পারে, তাই সঠিক কারণ নির্ণয় করা গুরুত্বপূর্ণ।
ডেঙ্গু সাধারণত Aedes মশার মাধ্যমে ছড়ানো ভাইরাসজনিত সংক্রমণ। টাইফয়েড হলো Salmonella Typhi ব্যাকটেরিয়াজনিত সংক্রমণ, যা সাধারণত দূষিত খাবার বা পানির মাধ্যমে ছড়ায়। আর দীর্ঘমেয়াদি জ্বরের পেছনে সংক্রমণ ছাড়াও বিভিন্ন প্রদাহজনিত, Autoimmune বা অন্যান্য শারীরিক কারণ থাকতে পারে।

ডেঙ্গুর ক্ষেত্রে দেখা যেতে পারে—

দীর্ঘমেয়াদি জ্বরে জ্বরের পাশাপাশি ওজন কমে যাওয়া, রাতের ঘাম, দীর্ঘস্থায়ী কাশি, দুর্বলতা, জয়েন্ট ব্যথা, পেটের সমস্যা বা শরীরের নির্দিষ্ট কোনো অংশে ব্যথা থাকতে পারে।
কী কী কারণে দীর্ঘমেয়াদি জ্বর হতে পারে?
দীর্ঘদিন বা বারবার জ্বরের বিভিন্ন কারণ থাকতে পারে। যেমন—
Tuberculosis (TB)

জ্বরের সময়কাল, জ্বরের ধরন, সঙ্গে থাকা উপসর্গ এবং রোগীর পূর্ববর্তী ইতিহাসের ওপর ভিত্তি করে কারণ অনুসন্ধান করা হয়।
কখন ডাক্তার দেখানো উচিত?
জ্বর কয়েকদিন ধরে চলতে থাকলে, বারবার ফিরে এলে বা জ্বরের সঙ্গে শ্বাসকষ্ট, দীর্ঘস্থায়ী কাশি, ওজন কমে যাওয়া, রাতের ঘাম, তীব্র দুর্বলতা, পেটের সমস্যা বা অন্য কোনো অস্বাভাবিক উপসর্গ থাকলে চিকিৎসকের পরামর্শ নেওয়া উচিত।
ডেঙ্গুর সম্ভাবনা থাকলে বিশেষ করে জ্বর কমে যাওয়ার সময়ও সতর্ক থাকা প্রয়োজন, কারণ এই সময় কিছু Warning Sign দেখা দিতে পারে।
কীভাবে Diagnosis করা হয়?
জ্বরের কারণ অনুযায়ী প্রয়োজনীয় পরীক্ষা নির্বাচন করা হয়। যেমন—

Dengue IgM/IgG, রোগের সময়কাল অনুযায়ী
Blood Culture

শুধু একটি পরীক্ষার ফলাফল দেখে সবসময় রোগের চূড়ান্ত Diagnosis করা যায় না। রোগীর উপসর্গ ও পরীক্ষার ফলাফল একসঙ্গে মূল্যায়ন করা হয়।
চিকিৎসা সম্পর্কে সাধারণ তথ্য
চিকিৎসা সম্পূর্ণভাবে জ্বরের কারণের ওপর নির্ভর করে।
ডেঙ্গুর ক্ষেত্রে পর্যাপ্ত Fluids, বিশ্রাম, জ্বর ও ব্যথা নিয়ন্ত্রণ এবং প্রয়োজন অনুযায়ী নিয়মিত Monitoring গুরুত্বপূর্ণ। Warning Sign বা গুরুতর ডেঙ্গুর লক্ষণ থাকলে হাসপাতালে চিকিৎসা প্রয়োজন হতে পারে।
টাইফয়েডের ক্ষেত্রে উপযুক্ত Antibiotic প্রয়োজন হতে পারে, তবে কোন Antibiotic ব্যবহার করা হবে তা চিকিৎসকের পরামর্শ ও প্রয়োজনীয় পরীক্ষার ওপর নির্ভর করা উচিত।
দীর্ঘমেয়াদি জ্বরের ক্ষেত্রে শুধু জ্বর কমানোর ওষুধ দিয়ে বারবার উপসর্গ নিয়ন্ত্রণ না করে মূল কারণ খুঁজে বের করে চিকিৎসা করা গুরুত্বপূর্ণ।
নিজে থেকে Antibiotic শুরু করা বা পুরোনো প্রেসক্রিপশনের ওষুধ পুনরায় ব্যবহার করা উচিত নয়।
কখন জরুরি চিকিৎসা প্রয়োজন?
জ্বরের সঙ্গে নিচের কোনো লক্ষণ দেখা দিলে দ্রুত জরুরি চিকিৎসা প্রয়োজন—

ডেঙ্গু ও টাইফয়েড কি একই রোগ?
না। ডেঙ্গু একটি ভাইরাসজনিত সংক্রমণ, আর টাইফয়েড Salmonella Typhi ব্যাকটেরিয়ার কারণে হয়। তাই Diagnosis ও Treatment এক নয়।
জ্বর হলেই কি Antibiotic খেতে হবে?
না। অনেক জ্বর ভাইরাসজনিত এবং সেখানে Antibiotic প্রয়োজন হয় না। সঠিক কারণ নির্ণয় করে প্রয়োজন অনুযায়ী চিকিৎসা করা উচিত।
ডেঙ্গু হলে কি Platelet কমলেই Platelet দিতে হয়?
সবসময় নয়। শুধু Platelet Count-এর একটি সংখ্যা দেখে সিদ্ধান্ত নেওয়া হয় না। রোগীর Bleeding, শারীরিক অবস্থা এবং অন্যান্য পরীক্ষার ফলাফলও বিবেচনা করা হয়।
দীর্ঘদিন জ্বর থাকলে কি অবশ্যই TB হয়েছে?
না। TB দীর্ঘমেয়াদি জ্বরের একটি সম্ভাব্য কারণ হলেও আরও অনেক সংক্রমণ ও অন্যান্য রোগের কারণেও দীর্ঘদিন জ্বর থাকতে পারে।
জ্বর কমে গেলেই কি ডেঙ্গুর ঝুঁকি শেষ?
না। ডেঙ্গুর ক্ষেত্রে জ্বর কমার সময় বা পরবর্তী সময়ে কিছু Warning Sign দেখা দিতে পারে। তাই চিকিৎসকের পরামর্শ অনুযায়ী Monitoring গুরুত্বপূর্ণ।

Dengue, Typhoid & Prolonged Fever
Fever is not a disease by itself; it is a symptom that can occur due to infection, inflammation or other medical conditions. Dengue, Typhoid and prolonged or recurrent fever can have different causes, making accurate evaluation important.
Dengue is a viral infection usually transmitted by Aedes mosquitoes. Typhoid is a bacterial infection caused by Salmonella Typhi, commonly transmitted through contaminated food or water. Prolonged fever can have many causes, including infections, inflammatory conditions, Autoimmune Diseases and other medical problems.
Common Symptoms

What Can Cause Prolonged Fever?
Prolonged or recurrent fever can have many possible causes, including—

When Should You See a Doctor?
Medical evaluation is recommended when fever persists for several days, repeatedly returns, or is associated with shortness of breath, persistent cough, unexplained weight loss, night sweats, severe weakness, abdominal symptoms or other unusual symptoms.
If Dengue is suspected, careful monitoring is particularly important even when the fever begins to settle, because Warning Signs can develop around this period.

Investigations are selected according to the suspected cause of the fever and may include—

Dengue NS1 Antigen
Dengue IgM/IgG depending on the timing of illness
Blood Culture
Culture-based testing when Typhoid is suspected
Urine Test and Urine Culture
Liver and Kidney Function Tests
Blood Glucose

TB Testing, such as Sputum or Molecular Testing, when appropriate

A single test result does not always establish the final diagnosis. Symptoms, examination findings and investigations are assessed together.
General Information About Treatment
Treatment depends entirely on the underlying cause of the fever.
For Dengue, adequate Fluids, Rest, Fever and Pain Control and appropriate Monitoring are important. Hospital treatment may be required when Warning Signs or severe Dengue develop.
For Typhoid, appropriate Antibiotic treatment may be required. The choice of Antibiotic should be guided by medical evaluation and, when appropriate, relevant testing.
For prolonged fever, repeatedly treating the fever without investigating its underlying cause may delay appropriate treatment. Identifying and treating the cause is therefore important.
Antibiotics should not be started on your own or reused from an old prescription.
When Is Urgent Medical Care Needed?
Seek urgent medical attention if fever is accompanied by—
Difficulty breathing
Severe abdominal pain
Repeated or persistent vomiting
Bleeding from the nose, gums, vomit, stool or elsewhere
Unusual drowsiness, confusion or loss of consciousness
Significant reduction in urine output
Severe weakness or dizziness
Cold, clammy skin or rapidly worsening condition
Seizures associated with fever

Frequently Asked Questions
Are Dengue and Typhoid the same disease?
No. Dengue is a viral infection, while Typhoid is caused by the bacterium Salmonella Typhi. Their Diagnosis and Treatment are therefore different.
Does every fever require Antibiotics?
No. Many fevers are caused by viruses and do not require Antibiotics. Treatment should be based on the identified or suspected cause.
Does a low Platelet Count in Dengue always require Platelet transfusion?
No. Treatment decisions are not based on a Platelet Count alone. Bleeding, overall clinical condition and other investigations are also considered.
Does prolonged fever always mean TB?
No. TB is one possible cause of prolonged fever, but many other infections and non-infectious conditions can also cause persistent fever.
Is the risk from Dengue over once the fever goes away?
Not necessarily. Warning Signs can develop around or after the time the fever settles. Appropriate Monitoring is therefore important.
