Coughing Blood
Blood in the sputum needs the cause identified quickly

Lung Infection
Tuberculosis (TB)
Bronchiectasis
Pulmonary Embolism
Tumour
Oxygen Saturation
Chest X-ray
Blood tests
Sputum
Molecular Test
CT Scan of Chest
Bronchoscopy
Diagnostic Investigation
Anti-TB Treatment
Procedure
Intervention
Frequently Asked Questions

Coughing Up Blood
Coughing up blood, blood-streaked sputum or reddish/brownish sputum is known as Hemoptysis. It can occur due to different conditions affecting the airways or lungs.
Small amounts of blood can sometimes occur after severe coughing or temporary airway inflammation. However, recurrent bleeding, larger amounts of blood, or bleeding associated with breathlessness, chest pain or other symptoms requires appropriate medical evaluation.
What Can Cause Coughing Up Blood?
Possible causes include—
Bronchitis or airway inflammation
Pneumonia or other Lung Infections
Tuberculosis (TB)
Bronchiectasis
Prolonged or severe coughing
Chronic lung diseases
Pulmonary Embolism
Some Tumours of the lungs or airways
* Bleeding disorders or the effects of Blood-thinning Medicines in some cases
The source of blood is not always the lungs. Bleeding from the nose, mouth or throat can sometimes mix with sputum. Identifying the actual source is therefore important.
How Can the Blood Appear?
The amount and appearance may vary. It can present as—
Small streaks of blood in sputum
Blood-stained mucus
Bright red blood
Repeated small episodes of bleeding
* A relatively larger amount of blood
Even a small amount should not be ignored if it keeps recurring.
When Should You See a Doctor?
It is advisable to seek medical evaluation even after a first unexplained episode of coughing up blood.
Prompt evaluation is particularly important if you have—
Recurrent blood in the sputum
Increasing amounts of blood
Persistent cough
Fever, night sweats or unexplained weight loss
Breathlessness or chest pain
Recurrent Lung Infections
A long history of smoking
A history of close contact with someone who has TB
How Is It Diagnosed?
Evaluation usually begins by determining how much blood is being produced, how long it has been occurring, the nature of the cough and whether other symptoms are present.
Depending on the situation, investigations may include—
Physical examination
Oxygen Saturation measurement
Chest X-ray
Blood tests
Sputum testing
GeneXpert or other Molecular Tests for TB
CT Scan of the Chest
Bronchoscopy
* Other Diagnostic Investigations when appropriate
The choice of investigations depends on the severity of bleeding, age, associated symptoms and suspected cause.
General Information About Treatment
Coughing up blood is a symptom, not a specific disease. Treatment therefore depends on the underlying cause.
Depending on the cause, treatment may include—
Appropriate treatment for an Infection
Anti-TB Treatment when TB is confirmed
Treatment for Bronchiectasis or Chronic Airway Disease
Medication or Procedures directed at the source of bleeding
Bronchoscopy or other Interventions when necessary
Hospital observation and treatment in selected cases
Do not start or stop Antibiotics or Blood-thinning Medicines on your own. Inform your doctor about any regular medicines you are taking.
When Is Urgent Medical Care Needed?
Seek urgent medical attention if you experience—
Large amounts of blood or continuous bleeding
Rapidly increasing bleeding
Difficulty breathing because of the bleeding
Severe breathlessness
Blue lips or face
Severe chest pain
Dizziness, fainting or marked weakness
Rapid deterioration of your condition
Frequently Asked Questions
Does coughing up blood mean I have TB?
No. TB is one possible cause, but Bronchitis, Pneumonia, Bronchiectasis and several other conditions can also cause blood in the sputum. Investigation is needed to identify the cause.
Is a small amount of blood always serious?
Not necessarily. However, unexplained or recurrent blood in the sputum should be medically evaluated.
Can blood from the nose or throat appear in the sputum?
Yes. Blood from the nose, mouth or throat can sometimes mix with sputum. Identifying the source is important.
Does everyone with Hemoptysis need Bronchoscopy?
No. Bronchoscopy is not required for everyone. It is considered when indicated by the patient's history, examination and imaging findings.
Can a smoker's coughing up blood simply be assumed to be Bronchitis?
No. Other important causes should also be evaluated when a smoker develops Hemoptysis.
