এজমা ও এলার্জি

এজমা ও এলার্জি খুবই সাধারণ সমস্যা। অনেক রোগী শ্বাসকষ্ট, বারবার কাশি, বুকে চাপ বা শ্বাস নেওয়ার সময় সাঁ সাঁ শব্দ নিয়ে আসেন। আবার কারও ধুলাবালি, ধোঁয়া, ঠান্ডা বাতাস বা কোনো নির্দিষ্ট পরিবেশে গেলেই সমস্যা শুরু হয় বা বেড়ে যায়।

সহজভাবে বললে, এজমায় শ্বাসনালী অতিরিক্ত সংবেদনশীল হয়ে যায়। বিভিন্ন trigger-এর কারণে শ্বাসনালীতে প্রদাহ ও সংকোচন তৈরি হলে শ্বাস নিতে কষ্ট হতে পারে। আর এলার্জি হলো কোনো নির্দিষ্ট পদার্থের প্রতি শরীরের অতিরিক্ত প্রতিক্রিয়া। অনেকের ক্ষেত্রে এলার্জি ও এজমা একসঙ্গে থাকতে পারে।

সাধারণ লক্ষণ

এজমা বা এলার্জির লক্ষণ সবার ক্ষেত্রে এক রকম হয় না। তবে সাধারণত যে সমস্যাগুলো দেখা যায়—

শ্বাস নিতে কষ্ট হওয়া
শ্বাস নেওয়ার সময় বুকে সাঁ সাঁ শব্দ হওয়া
বারবার কাশি হওয়া
দীর্ঘদিন ধরে কাশি থাকা
বুকে চাপ বা ভারী লাগা
রাতে বা ভোরের দিকে কাশি বা শ্বাসকষ্ট বেড়ে যাওয়া

নাক দিয়ে পানি পড়া বা নাক বন্ধ থাকা

অনেক সময় রোগী বলেন, “ডাক্তার সাহেব, ধুলার মধ্যে গেলেই আমার শ্বাসকষ্ট শুরু হয়” বা “রাতে শুলেই কাশি বাড়ে।” এই ধরনের তথ্য diagnosis-এর জন্য খুব গুরুত্বপূর্ণ।

কী কী কারণে বা Trigger-এ সমস্যা বাড়তে পারে?

সব রোগীর trigger এক নয়। আপনার যে জিনিসে সমস্যা বাড়ে, অন্য একজনের ক্ষেত্রে সেটি কোনো সমস্যা নাও করতে পারে।

সাধারণ trigger-এর মধ্যে রয়েছে—

ধুলাবালি
ধুলোর মাইট
ধোঁয়া ও সিগারেটের ধোঁয়া
বায়ুদূষণ
পরাগরেণু
ঠান্ডা বা শুষ্ক বাতাস

সর্দি-কাশি বা শ্বাসতন্ত্রের সংক্রমণ
অতিরিক্ত শারীরিক পরিশ্রম

তাই শুধু “আমার asthma আছে” জানলেই হবে না। কোন পরিস্থিতিতে আপনার সমস্যা বাড়ে, সেটাও বোঝা দরকার।

কখন ডাক্তার দেখানো উচিত?

মাঝে মাঝে সামান্য কাশি বা allergy হওয়া নিয়ে সবসময় চিন্তার কারণ নেই। কিন্তু সমস্যা যদি বারবার হয় বা দৈনন্দিন জীবনকে প্রভাবিত করে, তাহলে চিকিৎসকের পরামর্শ নেওয়া উচিত।

বিশেষ করে যদি—

বারবার শ্বাসকষ্ট হয়
শ্বাস নেওয়ার সময় সাঁ সাঁ শব্দ হয়
দীর্ঘদিন কাশি থাকে
রাতে কাশি বা শ্বাসকষ্টে ঘুম ভেঙে যায়

তখন শুধু সাময়িকভাবে কাশি বা শ্বাসকষ্ট কমানোর চেষ্টা না করে সমস্যার কারণটা ভালোভাবে মূল্যায়ন করা দরকার।

কীভাবে Diagnosis করা হয়?

শুধু শ্বাসকষ্ট বা কাশি আছে বলেই asthma বলা যায় না। একই ধরনের উপসর্গ অন্য অনেক রোগেও হতে পারে।

চেম্বারে প্রথমে আমি জানতে চাইব—সমস্যাটা কখন হয়, কতদিন ধরে হচ্ছে, রাতে বাড়ে কি না, ধুলা বা ঠান্ডায় বাড়ে কি না, ব্যায়ামের সময় হয় কি না এবং আগে এমন সমস্যা হয়েছিল কি না।

এরপর প্রয়োজন অনুযায়ী শারীরিক পরীক্ষা করা হয়।

Asthma সন্দেহ হলে Spirometry বা অন্য Lung Function Test করা হতে পারে। এতে ফুসফুস দিয়ে বাতাস চলাচলের ধরন এবং শ্বাসনালীর airflow-এর পরিবর্তন সম্পর্কে ধারণা পাওয়া যায়।

এলার্জির ক্ষেত্রে আপনার উপসর্গ ও history দেখে প্রয়োজন হলে Allergy Testing করা যেতে পারে। সবার জন্য একই পরীক্ষা প্রয়োজন হয় না।

চিকিৎসা সম্পর্কে সাধারণ তথ্য

এজমার চিকিৎসার উদ্দেশ্য শুধু আজকের শ্বাসকষ্ট কমানো নয়। আমরা চাই রোগটি দীর্ঘমেয়াদে ভালোভাবে নিয়ন্ত্রণে থাকুক, যাতে আপনার স্বাভাবিক জীবনযাপন ও দৈনন্দিন কাজকর্ম ব্যাহত না হয়।

চিকিৎসা আপনার বয়স, উপসর্গ, রোগ কতটা নিয়ন্ত্রণে আছে, lung function এবং trigger-এর ওপর নির্ভর করে।

একজন রোগীর জন্য যে ওষুধ বা inhaler প্রয়োজন, অন্য রোগীর জন্য সেটি একই নাও হতে পারে। তাই নিজের ইচ্ছায় inhaler শুরু, বন্ধ বা dose পরিবর্তন করা ঠিক নয়।

কখন জরুরি চিকিৎসা প্রয়োজন?

কখনো কখনো asthma-এর শ্বাসকষ্ট খুব দ্রুত বেড়ে যেতে পারে। এমন অবস্থায় বাসায় বসে অপেক্ষা না করে দ্রুত চিকিৎসা নেওয়া প্রয়োজন।

যদি—

খুব বেশি শ্বাসকষ্ট হয়
শ্বাসকষ্টের কারণে কথা বলতে কষ্ট হয়
শ্বাস নিতে প্রচণ্ড কষ্ট হয়
ঠোঁট বা মুখ নীলচে হয়ে আসে
খুব দুর্বল, অস্বাভাবিক ঘুমঘুম বা বিভ্রান্ত লাগে
prescribed reliever ব্যবহার করেও শ্বাসকষ্ট কমে না
* শ্বাসকষ্ট দ্রুত বাড়তে থাকে

তাহলে দ্রুত নিকটস্থ জরুরি বিভাগে যেতে হবে।

এজমা কি পুরোপুরি ভালো হয়?

অবশ্যই। asthma ভালোভাবে নিয়ন্ত্রণে থাকলে অধিকাংশ রোগী স্বাভাবিকভাবে হাঁটা, ব্যায়াম এবং অন্যান্য শারীরিক কাজ করতে পারেন।

তবে ব্যায়ামের সময় নিয়মিত শ্বাসকষ্ট বা সাঁ সাঁ শব্দ হলে বিষয়টি চিকিৎসককে জানানো উচিত।

Allergy থাকলেই কি asthma হবে?

না। allergy এবং asthma-এর মধ্যে সম্পর্ক আছে, কিন্তু allergy থাকলেই asthma হবে—এমন নয়।

Inhaler কি ক্ষতিকর বা আসক্তি তৈরি করে?

Inhaler হলো ওষুধ দেওয়ার একটি মাধ্যম, যার মাধ্যমে প্রয়োজনীয় medication সরাসরি শ্বাসনালীতে পৌঁছানো হয়। কোন inhaler প্রয়োজন এবং কতদিন ব্যবহার করতে হবে, তা রোগীর অবস্থার ওপর নির্ভর করে।

নিজের সিদ্ধান্তে inhaler বন্ধ করা বা পরিবর্তন করা উচিত নয়।

Asthma কি শুধু শীতকালে হয়?

না। ঠান্ডা বাতাস অনেকের trigger হতে পারে। তবে ধুলাবালি, ধোঁয়া, pollution, allergen, respiratory infection, exercise এবং অন্যান্য কারণেও asthma-এর সমস্যা বাড়তে পারে।

Allergy-এর trigger কীভাবে বোঝা যায়?

Asthma and allergy are very common problems. Many patients come to the chamber with shortness of breath, recurrent cough, chest tightness, or a whistling sound while breathing. For some people, symptoms become worse after exposure to dust, smoke, cold air, or a particular environment.

Simply put, asthma makes the airways overly sensitive. When the airways become inflamed and narrowed in response to certain triggers, breathing can become difficult. An allergy is an exaggerated reaction of the body's immune system to a particular substance. Asthma and allergy can also occur together in the same person.

Common Symptoms

Symptoms are not the same for everyone. Common symptoms include—

Shortness of breath
Wheezing or a whistling sound while breathing
Recurrent cough
Persistent cough
Chest tightness or heaviness
Cough or breathing difficulty that becomes worse at night or early in the morning
Shortness of breath during walking or exercise
Frequent sneezing

How Is Asthma Diagnosed?

Having shortness of breath or cough does not automatically mean that you have asthma. Similar symptoms can occur with many other conditions.

During a consultation, your doctor may ask when the symptoms occur, how long they have been present, whether they become worse at night, whether dust or cold air makes them worse, whether they occur during exercise, and whether you have experienced similar problems before.

A physical examination may then be performed when appropriate.

If asthma is suspected, spirometry or other lung function tests may be recommended. These tests help assess airflow through the lungs and identify changes in airway function.

For allergy-related symptoms, your symptoms and medical history are carefully considered. If appropriate, allergy testing may also be recommended. Not every patient needs the same test.

The goal of asthma treatment is not only to relieve today's breathing difficulty. We also want to keep the condition under good long-term control so that it does not interfere with your normal life and daily activities.

Treatment depends on your age, symptoms, level of asthma control, lung function, and relevant triggers.

Depending on your condition, treatment may include—

Inhaled medication
Controller medication
Reliever medication
Proper inhaler technique education
Allergy and trigger management
Necessary lung function or allergy-related testing
* Regular follow-up

The medication or inhaler needed by one patient may not be appropriate for another. Therefore, you should not start, stop, or change your medication or inhaler dose on your own.

When Is Emergency Treatment Needed?

Asthma symptoms can sometimes become severe and worsen rapidly. In such situations, you should not wait at home for the symptoms to settle.

Seek urgent medical attention if—

You have severe breathing difficulty
You have difficulty speaking because of breathlessness
You are struggling significantly to breathe
Your lips or face become bluish
You become extremely weak, unusually drowsy, or confused
Your prescribed reliever does not improve your breathing

Can I exercise if I have asthma?

Yes. When asthma is well controlled, most patients can walk, exercise, and remain physically active normally.

However, if you regularly develop shortness of breath or wheezing during exercise, discuss it with your doctor.

Does having an allergy mean that I will develop asthma?

No. Allergy and asthma can be related, but having an allergy does not necessarily mean that you will develop asthma.

Are inhalers harmful or addictive?

An inhaler is a way of delivering medication directly to the airways. The type of inhaler and duration of treatment depend on your individual condition.

You should not stop or change your inhaler without medical advice.

Does asthma only occur during winter?

How can I identify my allergy triggers?
